It’s Chainsaw vs. Sword as the bloody battle for Denji’s heart heats up. Denji is going to have to pull out all the stops against this crafty opponent. And when the dust settles, how will Special Division 4 move on from their losses?

The background of the coloured cover is light yellow with the title logo being rainbow coloured and author's name being black. The English-translated title is white and author's name is purple.

The cover depicts Denji, transformed into his Chainsaw Devil state, interlocked in combat with the Katana Man who is also transformed. The pair of them are falling as they fight and are surrounded by streamers of blood.
